Job summary

Independent Living Systems (ILS) seeks a Care Coordinator (IDD) to manage and coordinate comprehensive services for individuals with intellectual and developmental disabilities. You will assess needs, develop person-centered plans, and work with clinicians, families, and community resources to ensure timely access to care and adherence to program requirements.

This role emphasizes advocacy, seamless transitions between settings, and monitoring progress to adjust plans as needed.

Qualifications

IDD experience and strong collaboration with families required.
Bachelor's degree in social work or related field or equivalent.
LPN or RN licensure with relevant IDD experience preferred.
Proficiency in electronic health records (EHR) and basic computer applications.

Responsibilities

Serve as the primary point of contact for the member and their authorized representatives.
Assess member needs, identify care gaps, and assist in developing a person-centered support plan.
Monitor member’s progress and adjust care plans as necessary to address changing needs.
Coordinate services and care across the continuum and facilitate communication with providers and community resources.
Provide education and support on available resources and self-advocacy.
