Job summary

Independent Living Systems (ILS) is seeking a Care Coordinator (IDD) to join our team in Clearwater, FL. This role focuses on managing and coordinating comprehensive, person-centered care for individuals with intellectual and developmental disabilities, aligning with clinical guidelines and program requirements.

You will work with healthcare providers, families, and community resources to promote health, independence, and quality of life.

Qualifications

• Minimum of two years' experience working with individuals with intellectual developmental disabilities.
• RN or LPN licensure in Florida.
• Proficiency with EHR and basic computer applications.
• Education may be substituted on a year-for-year basis.

Responsibilities

• Serve as primary contact for the member and authorized representatives.
• Assess member needs, identify care gaps, and develop person-centered support plan.
• Monitor member progress and adjust care plans as necessary.
• Coordinate services across the continuum and communicate with providers and community resources.
• Provide education and support on available resources and self-advocacy.
• Maintain accurate documentation and ensure compliance with policies, regulations, and quality standards.
