Independent Living Systems (ILS) in Naples, FL, is seeking a Care Coordinator (IDD) to manage and coordinate comprehensive, person-centered care for individuals with intellectual and developmental disabilities. Qualifications

Two+ years of experience with individuals with intellectual developmental disabilities.
Bachelor’s degree in social services or related field, or RN/LPN with relevant experience.
Proficiency in electronic health records (EHR) and basic computer applications.

Responsibilities

Assess member needs and develop person-centered support plans.
Coordinate services and monitor progress across care settings.
Collaborate with healthcare providers, families, and community resources.
Advocate for members and ensure care aligns with guidelines and program requirements.
