Job summary

Independent Living Systems (ILS) is seeking a Care Coordinator (IDD) in Clermont, FL, to manage and coordinate comprehensive, person-centered care for individuals with intellectual and developmental disabilities. You will assess needs, develop plans, and liaise with providers and families to ensure seamless access to services and adherence to program requirements.

Qualifications

• Bachelor’s degree in social work, sociology, psychology, gerontology, or related social services field.
• LPN with a minimum of four years of relevant experience working with individuals with intellectual developmental disabilities.
• RN licensed to practice in the state of Florida.
• Proficiency in electronic health records (EHR) and basic computer applications.

Responsibilities

• Assist members and families in developing person-centered care plans.
• Coordinate services and supports across care settings.
• Monitor progress and adjust plans to changing needs.
• Collaborate with healthcare providers, social workers, families, and community resources.
• Maintain accurate documentation and ensure policy compliance.

Education

Bachelor’s degree in social work, sociology, psychology, gerontology, or related social services field
