Job summary

Independent Living Systems (ILS) in Fort Lauderdale, FL is seeking a Care Coordinator (IDD) to manage comprehensive, person-centered care for eligible individuals. You will assess needs, develop care plans, coordinate services, and liaise with providers and families to promote health, independence, and quality of life.

The role requires at least two years of relevant experience with IDD, a degree in a social services field (or RN/LPN licensure in Florida), and proficiency in EHR systems.

Qualifications

• Minimum of two (2) years of relevant experience with individuals with intellectual developmental disabilities.
• Proficiency in electronic health records (EHR) and basic computer applications.
• Bachelor’s degree in social work, sociology, psychology, gerontology, or related field; RN or LPN licensure in Florida.

Responsibilities

• Assess member needs and develop person-centered care plans.
• Coordinate services and supports across care settings.
• Serve as liaison between members, families, and care providers.
• Monitor progress and adjust plans to ensure quality and outcomes.
• Maintain documentation in compliance with policies and regulations.
