Qualifications

• Bachelor’s degree in Psychology, Social Science, or related field.
• Current CPR certification.
• Proof of eligibility to work in the United States.
• Current child abuse clearance and fingerprinting, as required.
• Ability to travel throughout the assigned service area; company vehicles are not provided.

Responsibilities

• Complete initial evaluations to assess client needs, appropriateness of services, and environmental safety.
• Participate in the development and implementation of ISPs.
• Coordinate services based on each client’s individual needs, goals, and expected outcomes.
• Provide monthly written summaries of client progress and services to Supports Coordinators/Administrative Entities.
• Maintain effective communication with clients, families, caregivers, staff, and Supports Coordinators.
• Review paraprofessional progress notes to ensure appropriate and effective service delivery.
• Educate paraprofessional staff on ISP expectations and service requirements.
• Monitor client status and changes in medical, family, and service needs.
• Conduct employee performance evaluations at 90 days and annually.
• Maintain client confidentiality and professional boundaries.
• Follow company policies, patient care standards, licensure requirements, and applicable state and federal regulations.
