Job summary

Aveanna Healthcare in the United States is seeking compassionate home health aides to provide one-on-one services to individuals with ID/DD in their homes and communities, following the Person Centered Plan. You will assist with daily living tasks and coordinate with the interdisciplinary team to meet goals.

Qualifications

• Experience with ID/DD individuals preferred, but not required.
• Proof of eligibility to work in the United States.
• High school Diploma or GED.
• Active Driver's License and current automobile insurance.
• Good organizational and communication skills.

Responsibilities

• Implement written goals from treatment plan with consumer 1:1 services including Personal Care, Respite, Home Supports, and Vocational Support.
• Instruct and reinforce target goals and outcomes according to consumer needs.
• Complete written documentation of services provided upon completion of visit as directed by QP.
• Accurately note consumer responses and results to staff interventions and assessments.
• Maintain professional conduct at all times.
• Assist in development of consumer treatment plan and goal revisions as directed by interdisciplinary team.
• Provide care in accordance with written instructions from professional staff.
• Report changes in patient condition or problems to QP immediately.
• Relay observations of patient status to QP.
• Be dependable and on time for assignments.
