Job summary

Qualifications

• Graduate from an accredited nursing program and hold NY state RN license (or CT if applicable).
• 1–2 years of professional nursing experience in related specialty preferred.
• Current BLS certification; additional certifications as needed.

Responsibilities

• Assess physiologic health status, document plan of care and outcomes.
• Coordinate care with physicians and health team members.
• Assess psychosocial health and patient education needs.
• Implement plan, administer treatments, monitor safety, and document care.
• Collaborate in discharge planning and referrals; provide family education.
