Job summary

Northwell Health in New York seeks a Registered Nurse to provide age-appropriate care using the nursing process across assessment, planning, implementation and evaluation.

You will collaborate with physicians and the health team, coordinate care, document accurately, support discharge planning, lead clinical activities, and educate patients and families. Requires 1–2 years' nursing experience, NYS license, and BLS.

Qualifications

• Graduate from an accredited School of Nursing; ADN required, BSN preferred.
• New York State RN license required; BLS certification.
• Minimum 1–2 years of professional nursing experience in the related specialty.

Responsibilities

• Assess the patient’s physiologic health status.
• Assess psychosocial health status and support patient and family.
• Identify patient outcomes and develop individualized goals.
• Plan and prioritize care; coordinate supplies, equipment, and meds.
• Evaluate care effectiveness and document outcomes.
• Collaborate with physicians and care team; delegate as needed.
• Perform Decubitus care and skin care as prescribed.
• Administer medications safely and educate patients.
• Educate patients and families; plan discharge and referrals.
