Job summary

Lutheran Health Network is seeking a Patient Care Technician (PCT) to provide high-quality, patient-centered care under RN/LPN supervision in Fort Wayne, Indiana. You will assist with activities of daily living, monitor and report patient data, and ensure a clean, safe care environment.

Your role includes supporting meals, hygiene, mobility, and documentation, while maintaining professionalism and effective communication with the healthcare team.

Qualifications

• 0-2 years of experience in an acute care setting or currently enrolled in a Nursing program preferred.

Responsibilities

• Assist nursing staff in delivering care and keeping environment safe and organized.
• Record patient data including vital signs and intake/output, report to RN/LPN.
• Support patients with meals, bathing, grooming, and toileting.
• Assist with patient positioning and mobility using aids.
• Collect urine/stool samples and perform finger-stick glucose monitoring.
• Communicate patient information to the care team and maintain professionalism.
• Maintain infection control protocols and proper PPE usage.
• Participate in performance improvement and risk management activities.
• Remain with patient for continuous observation if required.
