• Collect and record patient data including vital signs, intake/output, and calories; report findings to RN/LPN/LVN.
• Assist with meals, bathing, oral care, grooming, linen changes, skin care, elimination assistance, and catheter care.
• Assist with patient positioning, ambulation, and use of mobility aids.
• Document glucose monitoring and report results to RN/LPN/LVN.
• Communicate patient information effectively to the care team and maintain professionalism.
• Maintain infection control and PPE protocols to ensure patient and staff safety.
• Participate in performance improvement initiatives and risk management reporting.
