Job summary

Qualifications

• Collected patient data including vital signs and intake/output.
• Provided personal care: bathing, grooming, feeding, and hygiene.
• Monitored patient progress and assisted with mobilization and transfers.
• Supported discharge planning and care transitions with the nursing team.

Responsibilities

• Assist with data collection and reporting to RN.
• Provide direct patient care under RN supervision.
• Assist with activities of daily living and mobility as needed.
• Maintain a clean, safe, and orderly environment.
• Document care in the patient record.
