Job summary

NALHE is seeking a registered nurse to provide and coordinate comprehensive care for patients in collaboration with the health care team. You will use clinical judgment to assess needs, plan and implement care, and evaluate responses, while ensuring safety and quality at every step.

In this role you will document in the EMR, discharge planning, and work with nurses and other disciplines to optimize patient outcomes during hospitalization and beyond. Oregon-based facility experience preferred.

Qualifications

• Assessments across multiple organ systems and psychosocial factors.
• Develops individualized nursing plans of care.
• Delegates patient care appropriately to team members.
• Documents patient care and evaluates responses for quality improvement.

Responsibilities

• Assessment: performs multi-system assessments and discharge planning needs.
• Planning: develops and documents nursing care plans and educates patients.
• Implementation: administers medications per protocol and documents in EMR.
• Evaluation: evaluates patient response and participates in unit quality assurance.
