Qualifications

• Bachelor's degree or RN license required; social worker certification preferred.
• Two years in health or human services with one or more populations served.

Responsibilities

• Obtain information from clients and assess needs.
• Provide information aligned to client goals with a person-centered approach.
• Maintain knowledge of vocational, housing and community resources.
• Make referrals to other agencies for services as appropriate.
• Maintain relationships with human services and medical agencies.
• Provide educational presentations and participate in outreach.
