Job summary

Winnebago County, Wisconsin seeks an ADRC Specialist to inform and assist elderly and disabled adults about community resources and long-term care options. The role includes case management, counseling, and enrollment support in Wisconsin programs.

The position is full-time with a 37.5-hour workweek and standard 8:00 a.m. to 4:30 p.m. hours, based in the Human Services department. Prior social services experience is valued.

Qualifications

  • Bachelor's or master's degree in Social Work preferred.
  • AIRS certification within one year of hire.
  • Certification to administer the Wisconsin Long Term Functional Screen within one year.

Responsibilities

  • Collaboratively assess the needs, strengths, goals and outcomes of consumers and families.
  • Provide information and assistance to help consumers and families make informed long-term care decisions.
  • Help consumers access community resources and natural supports.
  • Conduct initial consumer-needs screenings using the online functional screening tool.
  • Provide long-term care options counseling and enrollment counseling.
  • Refer consumers and family members to appropriate services and assist with applications and follow-up.
  • Document case-management activities per division standards.
  • Give occasional public talks to educate groups about the ADRC and services.
  • Maintain current information about long-term living resources for the community.
