Supportive Living Solutions in Dakota County, MN is seeking an Entry‑level Case Manager to join our growing team in Eagan. The role offers a hybrid schedule, with up to two days remote after a 90‑day introductory period, and a typical caseload around 38 clients.

Qualifications

• Bachelor’s degree in health and human services OR bachelor’s degree in any field with 1 year of experience in the human services field.
• Ability to pass DHS background check.
• Driver’s license, reliable vehicle, vehicle insurance & clean driving record.

Responsibilities

• Serve a caseload of clients – our average is 38 clients per caseload, lower than industry standard.
• Complete new client intakes.
• Develop initial service plan with the client and legal representative.
• Coordinate services between providers and clients.
• Schedule in-person meetings with clients at least twice per year and make monthly phone contact with each client.
• Maintain quality documentation, such as case notes, service plans and service agreements.
