Job summary

Supportive Living Solutions in Duluth, MN, is seeking an Entry-level Case Manager to support a caseload of clients within our social services program. The role requires a 4-year degree in health and human services or related field and offers daytime hours, a hybrid work option, and competitive pay.

You will complete new client intakes, develop service plans, coordinate with providers, and maintain documentation.

Qualifications

• Bachelor’s degree in health and human services or a related field.
• Bachelor’s degree in any field plus 1 year of social work/care coordination experience.

Responsibilities

• Serve a caseload of clients with average 38 clients per caseload.
• Complete new client intakes.
• Develop initial service plans with clients and legal representatives.
• Coordinate services between providers and clients.
• Schedule in-person meetings at least twice per year and monthly phone contact.
• Maintain quality documentation (case notes, service plans, and service agreements).
