Job summary

Pinnacle Services is seeking a Case Manager for the Hennepin County Float to join a fast-paced, patient-centered team. You will work with waivers CADI, DD, BI, and AC/EW, handling 10–40 cases and collaborating with interdisciplinary teams.

The role requires a bachelor's degree in social work or a related field and a valid driver’s license. LSW licensure is preferred, with a hybrid schedule after training and strong growth opportunities.

Qualifications

• Bachelor's degree in social work, psychology, sociology, or related field.
• One year of full-time experience as a social worker/case manager/care coordinator in a social service agency.
• One year of full-time experience in education or treatment of persons served with developmental disabilities or related conditions.
• At least one course focused on developmental disabilities or transcript to verify coursework.
• Valid driver's license and insurance for vehicle.
• Successful background checks.

Responsibilities

• Assess areas where the person served needs assistance.
• Coordinate and monitor resources for the person served.
• Develop and manage service plans.
• Attend regular meetings with interdisciplinary teams.
• Document case notes for billable units.
• Manage 10–40 cases with a target to stay under 50 clients.
