The role involves working with clients on CADI, DD, or AC/EW waivers, coordinating resources, and developing service plans in a hybrid work setting after training and performance milestones.

Qualifications

• Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field.
• One year of full-time experience as a social worker/case manager/care coordinator in a public or private social service agency.
• One year of experience in education or treatment of persons served with developmental disabilities or related conditions
• At least one course focused on developmental disabilities required; transcripts provided.

Responsibilities

• Assess the areas of the individual's life where assistance is needed.
• Offer and coordinate resources for the persons served being supported.
• Monitor service delivery and work with providers to ensure the person's served needs are met.
• Service plan development.
• Attend regular in person meetings with persons served and their interdisciplinary teams.
• Case noting individual served related work to create billable units.
