Job summary

Accord is seeking a mission-driven Waiver Case Management (WCM) to join a team of Case Managers in supporting the health and human service needs of people with disabilities. The WCM will develop community-based plans, meet clients regularly, and authorize services within MN Health Care Programs, with MnChoices assessments participation.

Hybrid role; travel within the community and on-site training. Qualifications include a Bachelor’s Degree in Social Work (MN licensure if in social work) or

Qualifications

• Bachelor’s Degree in Social Work, Human Services, Psychology, or related field.
• If degree is in social work, current MN licensure is required.
• Bachelor’s degree in non-Human Services field must possess one year of related experience.

Responsibilities

• Develop a community support plan and goals for each person on an assigned caseload.
• Meet with each person regularly to evaluate progress and needed changes.
• Develop person-centered service agreements with the person to authorize services.
• Authorize services to meet the person’s assessed needs and goals.
