Pinnacle Services, Inc. is seeking a Case Manager for the Hennepin County area. The role focuses on person-centered planning and coordinating services for clients with diverse needs. You will manage a caseload with independence, collaborating with teams while maintaining client-driven goals.

The position requires a four-year degree in social work or related field and a valid license or progress toward licensure. A hybrid schedule may be available after training and performance milestones are met.

Qualifications

• Bachelor's degree in social work, psychology, sociology, or related field required or equivalent.
• One year of full-time experience as a social worker or case manager preferred.
• LSW or higher licensure preferred or ability to obtain licensure.

Responsibilities

• Assess areas where the individual needs assistance.
• Offer and coordinate resources for the persons served.
• Monitor service delivery and coordinate with providers.
• Develop and implement service plans.
• Attend regular in-person meetings with clients and interdisciplinary teams.
• Document case notes for billable units.

Education

Bachelor's in social work or related field

LSW or BOSW licensure

One year of social work/case management experience
