Job summary

24 Hour Home Care seeks a Community Supports Coordinator in El Segundo, CA to manage member utilization and reauthorization while coordinating with funding partners to ensure uninterrupted services.

The role involves proactive outreach to members and caregivers, documentation in Salesforce, and collaboration with health plans to resolve issues while maintaining HIPAA compliance. This is a hybrid, temporary position in California.

Qualifications

• High school diploma or GED required; bachelor’s degree preferred.
• 1–3+ years of customer service or other service-oriented experience.
• Experience with ticket management systems and processes.
• Proficiency with Microsoft Office, including Word, Excel, and Outlook, and experience with Salesforce.
• Ability to manage a high volume of communications and cases while meeting deadlines in a fast-paced environment.
• Reliable internet service required.

Responsibilities

• Monitor member utilization and authorized hours each billing cycle, proactively addressing underutilization, overages, and potential service disruptions.
• Manage utilization and reauthorization cases in Salesforce, ensuring required documentation is collected and extensions are completed in a timely manner.
• Partner with funding sources, health plans, and Case Managers to communicate member status, resolve authorization issues, and support continuity of services.
• Build and maintain relationships with members and caregivers, providing proactive outreach, resources, and support throughout ongoing service delivery.
• Respond to member and caregiver inquiries across phone, chat, text, and email, working to provide timely and effective resolution.
• Maintain accurate member records, including service utilization, authorization status, interactions, and required health plan updates.
• Collaborate across internal teams and external partners to resolve service issues, meet closed-loop referral and care integration requirements, and maintain HIPAA compliance.
