• Answer member and caregiver inquiries related to care plans and services.
• Communicate with care managers and practitioners to ensure continuity of care.
• Assist with service assessments/screenings and document member care needs.
• Document and maintain member records per state and regulatory requirements.
• Follow policies and standards; identify needs and make referrals to appropriate resources.
• Educate members about benefits and available resources.
• Perform other duties as assigned.
