Golden Gate Regional Center is seeking a Social Worker - Early Start Assessment to lead the assessment of applicants (0-36 months) applying for Early Start services. You will coordinate eligibility assessments and help develop Initial IFSPs within 45 days of referral, in a hybrid setup across client homes, the San Mateo office, and remote work.

The role requires Master’s or Bachelor’s in Social Work with related experience, strong interviewing and counseling skills, and ability to work with

Qualifikationen

• Master’s degree in Social Work preferred; BA in social work with 1 year experience acceptable.
• 2+ years working with developmentally disabled population.
• Ability to coordinate assessments and develop IFSPs within required timeframes.

Aufgaben

• Conduct initial Early Start applicant assessments and interviews.
• Coordinate CDER and IFSP development and timelines.
• Maintain confidential applicant records and prepare reports.
• Provide information and referrals to community resources.
• Collaborate with interdisciplinary team and families.
• Schedule meetings with team and applicants.
