Job summary

Qualifications

• Bachelor’s degree from an accredited college or university or Registered Nurse license.
• Experience with case management and documentation applications is preferred.
• Ability to drive and reliable transportation required.

Responsibilities

• Manages a caseload of varying ages, disabilities, and case complexity to improve quality of support and services.
• Develops community relationships and links resources to meet individual needs.
• Monitors and assesses quality and timeliness of services through ongoing reviews.
• Ensures documentation is completed within three business days of activity and maintains required billable hours.
• Keeps knowledge up-to-date via training and conferences.
• Fosters team unity and willingness to take on special assignments.
