Job summary

Keystone is seeking a Care Coordinator to deliver person-centered case management for elderly or disabled adults enrolled with Medica Health Plan. You will assess needs, develop care plans, and coordinate services in the Hennepin and Ramsey County area.

The role requires a Bachelor’s in Social Work and experience with Minnesota Waiver programs; a valid MN driver’s license is preferred. Hybrid work model with in-office Wednesdays.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Two years experience with Minnesota Home and Community Based Waiver Programs or MSC+/MSHO.
• Experience with seniors or disabled adults in community settings; ability to assess needs and link to resources.
• Proficient in computer skills including database and internet; able to type efficiently.

Responsibilities

• Provide person-centered case management services for Medica Health Plan enrollees.
• Complete community assessments to determine level of care and needs.
• Develop and monitor individualized care plans with client involvement.
• Coordinate services, document actions, and report to funding agencies.
• Maintain and organize case notes and required paperwork.
