Job summary

Bluestone-Physician-Services is seeking a Care Coordinator to empower Minnesota’s senior population to navigate their healthcare journey and live healthier, more independent lives. In this hybrid role, you’ll manage DHS and CMS requirements for MSHO/MSC+ members, combining home visits with remote work.

You will travel locally for in-home assessments and coordinate with care teams to ensure access to waiver services and community resources.

Qualifications

• MN RN or MN-LSW/LISW/LICSW eligibility required.
• Minimum 1 year experience with geriatric or disabled populations in case management/care coordination.
• Must have a valid driver’s license.

Responsibilities

• Coordinate face-to-face visits with members.
• Manage the Elderly Waiver program.
• Conduct annual PCA assessments and living tools for residents of Assisted Living.
• Review current health needs, set goals, and develop individualized care plans.
• Connect members with community resources and services.
