Keystone Community Services is seeking a Waiver Case Manager Supervisor – AC/EW to lead a team delivering Home and Community Based Waiver services in Ramsey and Hennepin Counties. The role includes staff supervision, training, and program development within a hybrid work model requiring three in-office days weekly.

The position reports to the Case Management Program Director and involves maintaining quality service, supervising case managers, and ensuring regulatory compliance.

Competenze

• Bachelor's degree in Social Work or related field required.
• 2 years of Case Management experience including working knowledge of case management methods/principals of social work practice.
• Understanding of professional boundaries, resources, supervision and interventions.
• Knowledge of case management issues for those experiencing serious mental illness or developmental disability.

Mansioni

• Lead and supervise Waiver Case Management staff for AC/EW programs.
• Oversee hiring, training, and intern management; develop program services and reporting.
• Maintain caseloads and ensure delivery of client services per Keystone standards.
• Develop CSSP plans based on MnChoices results and county allocations.
• Coordinate with Ramsey County DHS and internal/external partners.
