Keystone Community Services seeks a Waiver Case Manager Supervisor to lead the Disability Services team in Ramsey County and Hennepin County. Qualifikationen

• A bachelor's degree in Social Work or related field.
• 2 years of Case Management experience.
• Knowledge of case management issues for those experiencing serious mental illness or developmental disability.
• Understanding of professional boundaries and community resources.

Aufgaben

• Staff Direction: supervise and manage a team of Disability Services case managers.
• Program/Contract Management: review referrals, maintain training materials, ensure compliance.
• Case Management: provide person-centered services and develop CSSP based on MnChoices results.
