Job summary

Bluestone Physician Services is seeking a Care Coordinator to empower Minnesota seniors to navigate healthcare and live independently. You will balance DHS/CMS requirements, coordinate home visits, and connect members with waiver services and community resources.

The hybrid position combines remote work with local visits (Brooklyn Center/Minneapolis). Salary ranges from $65,000 to $75,000, commensurate with experience.

Qualifications

• Must hold a current MN social work license or RN license in Minnesota.
• Experience with geriatric or disabled populations in case management is preferred.
• Driver's license is required for travel to patient homes and facilities.

Responsibilities

• Coordinate face-to-face visits with members and their families.
• Manage DHS and CMS requirements for MSHO/MSC+ members.
• Develop individualized care plans and link to community resources.
• Assist with transitions of care and ensure safe discharge planning.
• Complete required documentation and collaborate with care teams.

Education

MN Licensed Social Worker license or equivalent

Minnesota RN license

Experience with geriatric or disabled populations in care coordination

Valid driver's license
