Job summary

Keystone Community Services is seeking a Care Coordinator in St. Paul, MN, to provide person-centered assessment, planning and advocacy for elderly or disabled adults enrolled with Medica Health Plan.

Hybrid work arrangement with in-office requirements three days a week and in-office Wednesdays. The role involves monitoring health and safety needs, coordinating services, and maintaining caseloads of 60–75 clients.

Qualifications

• Bachelor’s degree in Social Work or related field is required.
• Two years experience with Minnesota Home and Community Based Waiver Programs or MSC+/MSHO is preferred.
• Experience with seniors or disabled adults in community settings is preferred.
• Strong written and verbal communication skills and computer proficiency are required.

Responsibilities

• Provide person-centered case management services for Medica Health Plan enrollees.
• Complete assessments to determine level of care and needs, gathering relevant social and medical history.
• Develop and monitor person-centered care plans with client involvement.
• Coordinate and evaluate services to meet individual goals and adjust plans as needed.
• Maintain accurate case notes and documentation for funding entities.
