Job summary

Endeavor Health is hiring a Transitional Care Navigator to manage case and utilization review across care settings. You will coordinate transitions, educate patients and families, and partner with the healthcare team to optimize outcomes and costs.

The role requires a Bachelor’s in healthcare administration (RN preferred) with 3+ years in case management or discharge planning, and clinical nursing experience. On-site Arlington Heights presence with some remote coverage options, benefits included.

Qualifications

• Bachelor’s degree in healthcare-related field required.
• RN license with Illinois IDFPR required.
• 3+ years in utilization review, discharge planning, case management or disease management preferred.
• Home health or ambulatory care with high-risk patients beneficial.
• Strong communication and collaboration with stakeholders.

Responsibilities

• Guide high-risk patients through diagnosis, treatment, and follow-up within the health system.
• Document individualized care plans based on evidence-based guidelines.
• Collaborate with care team to ensure timely decisions and discharge planning.
• Coordinate across departments for smooth transitions and cost containment.
• Provide patient/caregiver education about resources, medications, and follow-up care.
• Arrange access to consultations and support services as needed.
• Support utilization management and monitor admission appropriateness.
• Travel to patient homes as necessary.
