Job summary

Centene Management Company LLC is seeking a registered nurse or nurse practitioner to perform hybrid in-home assessments in the Abilene, TX service area. You will evaluate complex/high-acuity members and develop long-term care service plans in collaboration with the care management team.

Responsibilities include coordinating with providers and family caregivers, updating care plans as conditions change, educating members on available services and benefits, and ensuring compliant, person-centered

Qualifications

• Graduate from an Accredited School of Nursing or a Bachelor's degree with 4–6 years of related experience.
• Bachelor's degree in Nursing preferred.

Responsibilities

• Assess and coordinate care plans for complex/high-acuity members.
• Educate members and families on services and benefit options.
• Coordinate with care management team to identify providers and resources.
• Monitor care plans and update as needed to meet member goals.
• Serve as liaison between members, families, and care providers.
