Qualifications

• Active RN license in good standing.
• URAC-recognized case management certification (CCM, CDMS, CRC, CRRN or COHN, COHN-S, RN-BC, ACM, CMAC, CMC).

Responsibilities

• Develop and implement individualized treatment plans with physicians/providers, employers, and patients.
• Coordinate in-person appointments and travel to assigned geography.
• Collaborate with employers and healthcare providers to ensure appropriate, cost-effective services.
• Manage workload and prioritize cases to achieve timely return-to-work goals.
