Genex Service LLC in Sacramento, CA is seeking a Medical Field Case Manager. This full-time, hybrid role requires a clinical nursing background and travel to in-person appointments within the assigned geography.

You will collaborate with treating physicians, employers, and patients to create and implement treatment plans, ensure cost-effective healthcare, and support maximum medical recovery and return-to-work goals with compassionate case management.

Competenze

• Associates or Bachelor's Degree in Nursing or related field.
• Active RN license in good standing required.
• URAC-recognized certification in case management (CCM/CDMS/CRC/CRRN or COHN/COHN-S, etc.).

Mansioni

• Demonstrate knowledge, skills, and competency in case management standards of practice.
• Advise on treatment plans using knowledge of injury types, medications, comorbidities, and job duties.
• Interview injured workers to assess recovery and determine occupational involvement.
• Collaborate with physicians/providers to tailor treatment plans to individuals.
• Coordinate with employers and physicians to modify duties for early return to work.
• Evaluate and adjust case goals based on progress and effectiveness.
• Independently manage workload and prioritize cases; attend appointments as needed.

Formazione
