Job summary

Enlyte is seeking a Field Case Manager (RN) for a full-time, hybrid role based in the Thornton, CO area with regular local travel for in-person patient appointments. Bilingual (English/Spanish) is highly desired.

The role involves collaborating with physicians, employers, and patients to build and implement effective treatment plans. You will assess injuries, advocate for appropriate care, and ensure cost-efficient services while supporting the patient’s return to work.

Qualifications

• RN license active and in good standing.
• 2+ years of clinical practice preferred.
• URAC-recognized certification in case management ( CCM, CDMS, CRC, CRRN or COHN, COHN-S, RN-BC, ACM, CMAC, CMC ) preferred.
• Travel to in-person appointments in assigned geography.
• Reliable internet connection.
• Strong written and verbal communication skills.

Responsibilities

• Collaborate with physicians to create and implement treatment plans.
• Coordinate care and ensure cost-effective, quality healthcare services.
• Schedule and attend in-person appointments with injured workers.
• Manage workload and prioritize cases independently.
• Communicate progress updates to customers and stakeholders.
