Job summary

Enlyte is seeking a Field Case Manager for a full-time, hybrid role based in the Bronx, NY area, requiring regular local travel for in-person patient appointments. You will work with treating physicians, employers, and patients to plan and implement treatment aimed at safe and early return to work, while ensuring cost-effective care.

The role emphasizes case management expertise, collaboration with medical teams, and the ability to navigate workers' compensation processes.

Qualifications

• Active Registered Nurse (RN) license required. Must be in good standing.
• URAC-recognized certification in case management (CCM, CDMS, CRC, CRRN or COHN, COHN-S, RN-BC, ACM, CMAC, CMC) preferred.

Responsibilities

• Demonstrate knowledge, skills, and competency in case management standards of practice.
• Use advanced knowledge of injuries, medications, comorbidities, treatment options, and job duties to advise on a treatment plan.
• Interview disabled persons to assess recovery and determine occupational relevance.
• Collaborate with physicians/providers to tailor treatment plans for individual patients.
• Work with employers and physicians to modify job duties to facilitate early return to work.
• Evaluate and modify case goals based on patient progress and treatment effectiveness.
• Independently manage workload, prioritizing cases and ensuring effective case management.
• Attend injured workers' appointments, prepare status updates for customers, and perform other duties as assigned.
