Job summary

Carilion Clinic in Roanoke, VA is hiring a Clinical Coordinator, RN for the Emergency Department Follow Up. The role is full-time with daytime hours and one remote day in a hybrid setup, working on-site at CRMH.

You will lead nursing processes, coordinate interdisciplinary care, and ensure effective transitions across the continuum of care while maintaining regulatory compliance and high-quality outcomes.

Qualifications

• Graduate of an accredited School of Nursing; bachelor’s degree preferred.
• Minimum of 3 years RN experience required.
• Current RN licensure in Virginia; BLS-HCP certification required or within 90 days of hire.
• Additional certifications may be required by department (ACLS, CCRN, ONC, Pain Management).
• Strong interpersonal, communication, teamwork, and critical thinking skills; must meet essential job functions.

Responsibilities

• Utilizes advanced nursing processes in decision-making, patient assessment, and care coordination.
• Oversees delivery of care and direct/indirect patient care within the department.
• Coordinates interdisciplinary care plans aligned with patient needs and guidelines.
• Performs assessments, facilitates referrals, and ensures appropriate transitions of care.
• Collaborates with physicians, nursing, social work, and ancillary services for coordinated care.
• Maintains accurate documentation and supports performance improvement initiatives.
