Qualifications

• Bachelor's degree in Health & Human Services or related field required.
• 1 year of social work, care coordination, case management, behavioral health, healthcare, or community-based services experience required.
• Experience with ID/DD and waiver services preferred.
• Experience coordinating treatment or recovery support services and advocating for patients preferred.

Responsibilities

• Provide in-person face-to-face assessments in Roanoke, VA.
