• RN: Baccalaureate Degree in Nursing; MSW preferred for Behavioral Health/Clinical Social Work.

• Provide clinical leadership and mentoring to RN Care Coordinators and Social Workers on the designated team.
• Facilitate patient care, problem-solving and stewardship of resources.
• Contribute to clinical, quality and financial performance of the Department.
