Job summary

Acentra Health, LLC is seeking a RN Clinical Assessor (Personal Care) to join our team in West Virginia. The role includes medical eligibility assessments for WV Medicaid recipients, adherence to guidelines, and data entry in Atrezzo within timelines.

The position requires extensive local travel and periodic travel outside the catchment area. This hybrid role combines remote work with field assessments and quarterly in-office meetings.

Qualifications

• Valid, unrestricted RN license for WV or compact license.
• ASN degree from an accredited college; BSN preferred.
• Valid WV driver license and reliable transportation.
• Willingness to travel within and outside catchment area.
• Organized with excellent customer service and communication skills.
• Ability to assess medical conditions and use common software tools.

Responsibilities

• Document contacts, schedule appointments, and travel for assessments.
• Participate in inter-rater reliability activities and denials hearings.
• Contact physicians/facilities to confirm information.
• Assess eligibility for PCS using approved tools and enter data timely.
• Ensure PCS care is provided based on beneficiary needs.
• Maintain privacy and adhere to HIPAA and policies.
• Engage families/caregivers during assessments and guide PCS providers.
• Submit assessments via state-approved interfaces and attend meetings.
