Job summary

Acentra Health is seeking an RN Clinical Assessor (Personal Care) to join our field-focused team in West Virginia. This hybrid role combines remote work with in-field assessments and quarterly in-office meetings.

Travel across the catchment area is required to conduct Pre-Admission Screenings and determine medical eligibility for PCS and the Aged & Disabled Waiver programs. The position requires an active WV RN license, ASN (BSN preferred), and travel flexibility.

Qualifications

• RN license valid and unrestricted in WV or a compact state.
• ASN degree required; BSN preferred.
• Valid West Virginia driver's license and reliable transportation.
• Willingness to travel to homes and facilities within the catchment area.
• Strong organizational, customer service, and interpersonal skills.
• Proficient computer skills with Outlook, Word, Edge, and Chrome.

Responsibilities

• Document contacts, schedule appointments, and travel to conduct Pre-Admission Screening assessments on time.
• Participate in inter-rater reliability activities and denial hearings as needed.
• Contact physicians and facilities to confirm medical information.
• Assess beneficiaries for PCS eligibility using state-approved tools.
• Ensure PCS are provided based on unmet needs and underlying conditions.
• Maintain privacy and dignity of individuals undergoing assessments.
• Conduct assessments within established timeframes and involve family members as needed.
• Guide beneficiaries to select PCS providers and submit assessments via state interfaces.
• Participate in mediation/appeals and attend staff meetings and peer reviews.
• Adhere to HIPAA, Privacy, and Security Rules in all activities.
