Job summary

Acentra Health seeks an RN Clinical Assessor to perform needs-based eligibility determinations for Medicaid-funded personal care services in home or community living settings. Hybrid role with face-to-face visits in Gaston and Cleveland counties; travel up to 60 miles from home base and potentially longer days.

The RN will assess level of care and ensure services are provided on a needs basis, while maintaining privacy and dignity of clients, and coordinating with families and care providers.

Qualifications

• RN license for NC or compact multi-state.
• Valid driver’s license and reliable transportation.
• 2+ years of home care experience.
• Proficiency with Microsoft Office Suite.

Responsibilities

• Conduct assessments to determine PCS eligibility using state-approved tools.
• Ensure PCS on a needs basis based on unmet needs.
• Maintain privacy and dignity of individuals during assessments.
• Complete assessments within established timeframes.
• Interview family members and informal caregivers.
• Guide Beneficiaries to select PCS providers.
• Conduct service plan reviews as needed.
• Submit assessments using state-approved interface.
• Participate in mediation and appeal processes.
• Respond to state inquiries about assessments.
• Attend staff meetings and conduct case consultations/internal audits.
• Provide initial eligibility assessments for 1915(c) HCBS program and annual/change assessments.
• Adhere to corporate policies including HIPAA.
