Job summary

Acentra Health is seeking a RN Clinical Assessor (Personal Care) in West Virginia to educate and perform medical eligibility assessments for WV Medicaid recipients in the Personal Care and Aged & Disabled Waiver programs. The role requires extensive local travel within the catchment area; combines remote work with field assessments and quarterly in-office meetings.

Applicants should reside in designated WV counties and be prepared for travel.

Qualifications

Valid RN license for West Virginia or compact license.
ASN degree from an accredited college/university; BSN preferred.
Valid West Virginia driver's license and access to reliable transportation.
Willingness to travel to homes and facilities within the catchment area.
Highly organized with excellent customer service and communication skills.
Proficient computer skills: Outlook, Word, Edge, Chrome.

Responsibilities

Document contacts, schedule appointments, and travel to conduct and document Pre-Admission Screening assessments within prescribed timelines.
Participate in inter-rater reliability activities and denial hearings as needed.
Contact physicians and facilities to confirm medical information.
Assess PCS eligibility using state-approved tools.
Ensure PCS are provided on a needs basis based on beneficiary needs.
Maintain privacy and dignity per HIPAA standards.
Complete assessments within established timeframes; involve family/ caregivers as needed.
Guide beneficiaries to select PCS providers and submit assessments via state interfaces.
Participate in mediation and appeals processes; attend staff meetings and peer reviews.
