Acentra Health seeks an RN Clinical Assessor (Personal Care) to join our team in West Virginia. This hybrid role combines remote work with field assessments and quarterly in-office meetings.

Expect extensive travel within the assigned catchment area and occasional travel outside it. Responsibilities include performing medical eligibility assessments for WV Medicaid recipients, coordinating with physicians, and documenting assessments in Atrezzo within required timelines.

Qualifications

• Valid, unrestricted RN license for West Virginia or compact license.
• ASN degree; BSN preferred.
• Valid WV driver's license and reliable transportation.
• Willingness to travel within catchment area and beyond for meetings.

Responsibilities

• Document contacts, schedule and travel for Pre-Admission Screening assessments.
• Ensure assessments are completed within prescribed timelines.
• Coordinate with physicians and facilities to verify medical information.
• Assess eligibility using state-approved tools for PCS and Aged & Disabled Waiver.
• Maintain privacy and dignity of beneficiaries during assessments.
• Attend staff meetings and participate in case consultations and audits.
