CVS Health is seeking a Case Management Coordinator to join our Aetna team, guiding members by coordinating appointments, benefits, and education through care management tools. You will contribute to better health outcomes for dual eligible populations while working from home across Illinois.

Ideal candidates have RN licensure, 3–5 years of clinical experience, and strong collaboration skills to support members, providers, and care teams in a resourceful, compliant environment.

Formación

• Requires Illinois residency and ability to travel up to 50-75% in and around Kane and DuPage Counties, IL.
• Minimum 3–5 years of clinical practical experience.
• Strong independent work capability and virtual collaboration.
• Proficient with MS Word, Excel, Outlook, and PowerPoint; familiarity with proprietary applications.
