Qualifications

• Active RN license to practice in North Carolina.
• Minimum of 2 years of RN experience.
• Graduation from a state-accredited school of professional nursing.
• Magnet hospitals: BSN required or must be enrolled in an accredited program with degree within specified timelines.

Responsibilities

• Conduct comprehensive care manager assessments using standardized tools to evaluate patients’ functional status and psychosocial needs.
• Collaborate with providers and families to create individualized, evidence-based care plans.
• Identify barriers to care, risk levels, and develop interventions to optimize health outcomes.
• Coordinate tasks and interventions across care settings; advocate for patients to ensure timely care.
• Coordinate care transitions and participate in multidisciplinary care team meetings; ensure accurate documentation and billing as applicable.
