Job summary

Advocate Health in North Carolina seeks an experienced Registered Nurse with a BSN to join the care management team. You will coordinate care across patients, families, primary and hospital teams, and community resources to overcome barriers and ensure timely delivery of quality, cost-efficient care.

You may work embedded in a provider office or telephonically with the care team. You will manage a CM caseload, document for MCO and regulatory requirements, and partner with at-risk patients

Qualifications

• Registered Nurse license required in the state where the team member practices.
• Bachelor's Degree in Nursing or related field.
• Typically 5 years of clinical nursing experience or 1-2 years in care management.

Responsibilities

• Facilitates communications among patient/family and care teams to resolve barriers to care.
• Collaborates with primary, hospital and post-acute teams to ensure quality, efficient care.
• Uses evidence-based approaches to engage patients and families and set SMART goals.
• Manages CM caseload and documents requirements for MCO and regulations.
• Partners with at-risk patients through diagnosis, treatment and follow-up.
• Coordinates patient information and referrals to ensure smooth transitions of care.
