Description

Schedule: 40 hours per week with flexible schedule options available, including weekend rotation, holiday rotation, no nights, no on-call.

We seek experienced RNs who have at least 2 years of experience, preferably in Adult Acute Care, with interest in discharge processes, patient advocacy, team environment, and desire to grow as a Care Manager.

Responsibilities

• Conduct comprehensive patient assessments using standardized tools.
• Develop and implement individualized, evidence-based care plans.
• Apply risk-segmentation tools to evaluate patient risk and create mitigation strategies.
• Coordinate and facilitate tasks/interventions to support care plans.
• Advocate for patient needs during multidisciplinary team meetings.
• Coordinate care transitions across settings.
• Communicate with payers to optimize reimbursement and approvals.
• Accurately document and bill services in compliance with regulations.
• Provide longitudinal care as part of the medical home.

Qualifications

• Registered Nurse with valid NC license.
• Minimum 2 years of RN experience.
• Strong knowledge of risk segmentation, patient assessment, care planning, transitions, and payer communication.
• Excellent communication, collaboration, problem solving skills.
• Ability to work independently and in multidisciplinary teams.
• Proficiency in EHR and relevant software.

Education

• Graduation from a state accredited nursing program.
• BSN preferred; enrolling to BSN within 4 years of hire accepted.
• Master’s degree accepted within 7 years of hire.
