Qualifications

• Graduation from a state accredited school of professional nursing.

Responsibilities

• Conduct comprehensive care manager assessments using standardized tools to evaluate patients' functional status and psychosocial support.
• Collaborate with providers and families to develop individualized, evidence-based care plans.
• Identify barriers to care and develop interventions to address them.
• Coordinate and facilitate necessary tasks and interventions to support care plans.
• Collaborate with providers, community resources, and payers to ensure seamless coordination of care and timely approvals.
• Advocate for patients and participate in quality improvement initiatives.
• Coordinate care transitions across settings to ensure continuity of care.
• Communicate with healthcare teams and payers to optimize reimbursement and services.
• Accurately document and bill for services per regulatory requirements.
